Flank pain or back pain

Not everything that hurts in the back is coming from the back.

Back pain from the spine changes with position and movement. Flank pain from an organ such as a kidney does not. It often comes in waves, and it tends to bring fever, nausea or urinary change. That position question is the single most useful way to tell them apart.

A handful of organ problems show up as back pain and get treated as musculoskeletal (muscle and bone) pain for weeks. The features that separate them are not subtle once you know to ask.

How can you tell if flank pain is muscular or from an organ?

Mechanical spinal pain has a relationship with position and movement. Something makes it worse, something makes it better, and there is a posture that gives relief. That relationship is the single most useful screening question in the whole examination.

Visceral pain (pain from an organ) does not behave that way. It is present no matter the position. It may come in waves. It often comes with whole-body signs. No amount of shifting in the chair changes it.

GET SEEN THE SAME DAY

  • Severe one-sided flank pain in waves, unrelieved by any position, often with nausea — classically a kidney stone.
  • Back or flank pain with fever, chills or burning on urination — possible kidney infection.
  • Blood in the urine with flank pain.
  • Sudden severe tearing back or abdominal pain, most of all over sixty with vascular risk factors, is an emergency and not a clinic call.
  • Upper back or between-the-shoulder-blade pain with chest tightness, sweating or breathlessness is a cardiac assessment now.

None of these is a spine appointment. Some are an emergency department.

What organ problems can feel like back pain?

  • Gallbladder disease — right upper abdomen and right shoulder blade, worse after fatty meals.
  • Pancreatitis — upper abdominal pain boring straight through to the mid back, often eased by leaning forward.
  • Endometriosis — cyclical low back and pelvic pain tracking the menstrual cycle. Frequently dismissed for years.
  • Abdominal aortic aneurysm (a bulge in the main artery of the belly) — deep constant back pain, sometimes with a pulsing feeling in the belly, in older patients with vascular risk.
  • Shingles, before the rash appears — burning in a band on one side that does not cross the midline.

How does a back pain visit screen for organ pain?

Every intake here asks the position question. It asks about fever, weight loss and urinary change. It asks what a patient’s other doctors are already treating them for. That is not thoroughness for its own sake. It is the cheapest way to avoid spending three months injecting a spine that was never the problem.

Where the story fits a mechanical pattern, we get on with finding which structure. Where it does not, we say so and send it on rather than treat the region that happens to be our specialty.

Frequently asked questions

How do I know if it’s back pain or my kidneys?

Kidney pain typically sits higher and more to the side than lumbar pain. It comes in waves rather than tracking movement. It often brings urinary symptoms, fever or nausea. It is not relieved by lying still.

For the spinal causes of pain on one side, read one-sided low back pain.

Can constipation cause back pain?

It can produce real referred low back discomfort (pain felt in the low back but coming from somewhere else). It is worth resolving before concluding anything about the spine.

My pain is worse after eating. Is that my back?

Very unlikely. A relationship with meals points to the digestive tract and deserves that workup instead. Visceral referral is a real consideration higher up the spine.

When should I worry about flank pain?

Get seen the same day for severe one-sided flank pain that comes in waves and no position relieves. That holds most of all with nausea, which is classically a kidney stone. The same goes for back or flank pain with fever, chills or burning on urination, and for blood in the urine. Sudden, severe tearing back or abdominal pain is an emergency, not a clinic call.

What can mimic flank pain or back pain?

Several organ problems pass for back pain. Gallbladder disease hurts in the right upper abdomen and right shoulder blade, worse after fatty meals. Pancreatitis bores straight through to the mid back and often eases when you lean forward. Endometriosis tracks the menstrual cycle. An abdominal aortic aneurysm gives deep, constant back pain. Shingles burns in a one-sided band before the rash appears.

Should I see a spine doctor or go to the emergency room?

It depends on whether your pain behaves mechanically. Spine pain has a position that makes it worse and one that gives relief. That belongs in a spine clinic. These kinds of pain are not a spine appointment: pain that nothing changes, pain with fever or urinary change, tearing pain, or upper back pain with chest tightness, sweating or breathlessness. Some of those need the emergency department now.
